The Congregational Health Partnership (CHP) is a partnership between local congregations and Lakeland Regional Health designed to empower faith communities to address the unique health needs of their congregation.

About the Program

How does the program work?
CONGREGATION + CLERGY + CONGREGATION LIAISON + LAKELAND REGIONAL HEALTH = FAITH AND HEALTH
It is just that simple. LRH partners with your congregation to come up with a plan that will benefit your members and improve their health.

How much does it cost to join the program?

The program is free. All we ask is that you provide a liaison from your church who works closely with Lakeland Regional Health to identify educational opportunities and organize health-centered experiences for your congregation.

How much time would we have to devote to the program?
It is completely up to you! You can be as involved as you choose.

What types of congregations does LRH work with?
All types. The opportunity to participate in the Congregational Health Partnership program is open to congregations of all faiths.
