In order to obtain your medical records, please contact the Medical Records department at 863.687.1067.
Please note that State and Federal laws permit fees to be charged for copies of medical records that are not requested by healthcare providers for direct patient care. To reduce costs incurred, patients should consider requesting specific information rather than a complete record.
In addition, LRH offers patients free access to the LRH Patient Portal, which provides secure online access to their healthcare information.
