What We Need from You:


When you call, we will ask for patient and clinical information. Please provide us with the following information:

  • Your name and contact number
  • Referring provider’s name and contact number
  • Patient’s name, DOB, SSN, diagnosis, and other relevant clinical information
  • Specialty requested
  • Level of care needed
  • Mode of transportation

Our Team and Our Process

The Lakeland Regional Health Transfer Center is staffed by our highly skilled registered nurses. When you call, one of our nurses will:

  • Identify the appropriate clinical setting and specialist for your patient based on capability, capacity, and patient preference.
  • Facilitate a recorded call between the referring and accepting provider.
  • Complete a registration.
  • Organize the bed assignment.
  • Assemble necessary clinical details for hospital admission.
  • Provide admission information to the referring provider.
  • Answer any questions throughout the process.
  • Facilitate physician-to-physician consults, when necessary.
