Anticoagulation Management Service

Anticoagulation Management Service at Lakeland Regional Health offers therapy management for patients on warfarin (Coumadin, Jantoven) with a focus on maximizing safety, quality, and outcomes. This service gives warfarin patients a consistent method for assessment, monitoring, and education.

Patient Eligibility

Most patients are seen in the office where testing is completed via point-of-care (POC) finger stick with results provided during the visit. Venipuncture (blood draw) or home POC testing is available for eligible patients.

Eligibility for home POC testing is based on insurance, warfarin duration of therapy, and compliance with warfarin monitoring. Patients who are established with the clinic or new to warfarin can expect the first office visit to last 30 minutes, as full education is provided. Established patient appointments last 15 minutes.

 

What to Expect

As a patient, you can expect to be asked the following questions at every point of contact:

  • What dose of warfarin are you currently taking?
  • Have you missed any warfarin doses or taken extra doses?
  • Any falls or injuries?
  • Scheduled procedures or surgeries?
  • Any bleeding symptoms including nose bleeds, gum bleeds, red or brown urine, or dark tarry stool?
  • Changes in prescription medications?
  • Use of any over-the-counter medications, supplements, or herbal products?
  • Changes in dietary intake of vitamin K or green vegetables?
  • Intake of cranberry products?
  • Any alcohol, tobacco, or use of illicit drugs?

To be managed by the Anticoagulation Management Service, patients must be referred by an LRH primary care provider or LRH specialty provider involved in their ongoing care, including decisions about their long-term anticoagulation therapy plan of care.

The Anticoagulation Clinic is not able to accept patients with Ambetter insurance.

Warfarin is an anticoagulant, or blood thinner, that prevents blood clots. Warfarin does not have standard dosing and requires routine monitoring with a blood test called INR to ensure safety and efficacy. Sometimes the INR will fluctuate due to numerous outside factors interfering.
