At Lakeland Regional Health, our outpatient diabetes services empower individuals and families with the knowledge, tools, and support needed to manage diabetes confidently and live well.

Living with diabetes can feel overwhelming, but you don’t have to manage it alone. Lakeland Regional Health’s Outpatient Diabetes Services provide comprehensive, evidence-based education and support for individuals living with prediabetes or diabetes.

Our team partners with you to build practical skills, set achievable goals, and create a care plan that fits your lifestyle – whether you’re newly diagnosed, learning how to eat well, or working to prevent complications.

Our Services

Education is provided by Certified Diabetes Care and Education Specialists (CDCES) and Registered Dietitians (RD) using nationally recognized standards of care.

Diabetes Self-Management Education & Support (DSMES)

Our DSMES program is designed to help individuals:

  • Understand their diabetes diagnosis
  • Learn how food, activity, and medications affect blood sugar
  • Build confidence in daily self-management
  • Reduce the risk of diabetes-related complications

Education is provided by certified diabetes care and education specialists and registered dietitians using nationally recognized standards of care.

Individual Diabetes Education

One-on-one sessions tailored to your needs, goals, and learning style. These visits may include:

  • Blood glucose monitoring
  • Medication and insulin education
  • Nutrition counseling
  • Lifestyle and behavior change strategies
  • Problem-solving for real-life challenges

Group Diabetes Education

Group classes provide a supportive environment to learn alongside others managing diabetes. Topics may include:

  • Understanding diabetes
  • Healthy eating and meal planning
  • Physical activity
  • Preventing complications
  • Coping and stress management

Diabetes Prevention Program (DPP)

Prediabetes is more common than many people realize, affecting an estimated 88 million adults in the U.S., and many individuals don’t experience symptoms, making early action essential. Lakeland Regional Health’s Diabetes Prevention Program (DPP) is designed specifically to help individuals reduce their risk of developing type 2 diabetes through sustainable, achievable lifestyle changes. 

The yearlong, evidence‑based program provides:

  • Supportive coaching from trained lifestyle experts
  • Guidance on healthy eating, physical activity, and stress management
  • Tools and resources to help you build long‑term habits
  • A community of peers working toward similar health goals 

By participating in the DPP and applying the strategies learned, individuals can significantly lower their chances of developing type 2 diabetes and may even reverse a prediabetes diagnosis.

Our outpatient diabetes services support:

  • Adults with type 1 or type 2 diabetes
  • Individuals who are newly diagnosed or living with diabetes long-term
  • Patients transitioning from inpatient to outpatient care
  • Individuals with prediabetes
  • Those managing diabetes during pregnancy (as referred)

Patients referred by primary care providers or specialists

  • Lakeland Regional Health Grasslands Campus (must be referred by an LRH provider)
  • Lakeland Regional Health Medical Center
  • A referral may be required depending on your insurance plan
  • Most major insurance plans are accepted
  • Self-pay options may be available

Diabetes Support Group

Managing diabetes is easier when you don’t do it alone. Our Diabetes Support Group brings together individuals living with diabetes, their caregivers, and family members for education, encouragement, and practical guidance.

Each session is led by members of our diabetes education team and covers topics such as nutrition, activity, medications, emotional health, prevention of complications, and everyday problem-solving. Participants are welcome to ask questions, share experiences, and build community.

Who Can Attend

Anyone living with diabetes or supporting someone who does is welcome to join. Admission is free and no registration is required.
