Accessing your child's medical information

The MyChart Patient Portal is an online tool that provides medical information about care provided at Lakeland Regional Health and connects you to your health care team.

MyChart is an online tool that provides your medical information and connects you to your health care team. It is convenient, secure and free.

Your provider can help you sign up for MyChart. Once you are enrolled, you can access your Lakeland Regional Health medical information in MyChart from your smartphone, tablet or computer. Click here to visit our sign up page or download the app from the App Store or Google Play store.

With MyChart you can:

• See all or portions of you/your child’s medical record, including some test results, scans and images depending on the age of the child

• Monitor health care provided

• View and pay bills

• Send secure messages to some members of the health care team

• Request prescription renewals

• Request an appointment

MyChart for Children

While making the transition to MyChart from our previous platform, there are a few things that Parents or Guardians who use MyChart should be aware of:

24 months of lab results and other health information like immunizations, medications, problems, allergies from our previous electronic medical record (EMR) system were transferred to MyChart. If you need older medical records, you can request medical records from our Health Information Management (HIM) department by clicking here.

MyChart for Children 12 or Younger:

Parents/guardians can request a proxy account and will have full access to the child’s MyChart information.

MyChart for 13- to 17-Year-Old Children:

Parents or guardians accessing information about a patient ages 13-17 will only have limited access to MyChart, with the ability to view/update insurance information, allergies, immunizations, and pay bills online. This limited access reflects privacy laws and encourages teens to feel comfortable sharing health concerns with their care team. Teens aged 13-17 are encouraged to establish their own MyChart account, where they can see their full information.

MyChart For Young Adults Ages 18 and Older:

When the patient turns 18, the parent/guardian’s proxy access automatically ends. The patient may complete and sign an adult proxy authorization to continue their parent/guardian’s proxy access.

Signing Up for MyChart!

All access to records through MyChart requires a MyChart account. Parents and guardians can be granted “proxy access” to the accounts for their children.

How to Sign Up:

Reach out to the provider’s office and they can help you activate your MyChart account.

Please call our MyChart patient support line at 863.284.1519.

  • Once a child turns 13, the proxy access that parents had prior to their child turning 13 automatically transitions to more limited access to reflect privacy laws and encourages teens to feel comfortable sharing health concerns with their care team. Teens aged 13-17 are encouraged to establish their own MyChart account.

  • Your teen (13-17 years old) may request proxy access for you. Please call our MyChart patient support line at 863.284.1519.

• A patient 18 years of age or older must create their own MyChart account by clicking here and selecting “Sign Up Now” or by calling their provider.

• Once the 18-year-old patient has their own MyChart account, they can log in and click “Share my Record” under the menu to complete the online form and submit.

• Once the form is submitted it will be reviewed and the patient will be informed of the status of their request.

• If the new proxy does not have a MyChart account, one will be created for them.
