Requesting Medical Records During the COVID-19 Pandemic

To ensure the safety of our patients and families, our Medical Records office is temporarily closed for in-person record requests.


PARENTS REQUESTING RECORDS FOR CHILDREN (under 18):

You do not have to provide supporting documentation at this time. However, if you are not the parent listed in the child’s record, a representative will contact you for additional information


REQUESTING COVID-19 TEST RESULTS: 

If you have been tested for COVID-19 at one of Lakeland Regional Health’s testing sites, a text message with results will be sent to the number of record. If a full report is needed, a digital or paper copy can be requested as shown below.


There are two ways you can request your medical records:

Electronically-

Requesting your records electronically is quick and easy through our medical records digital intake. On the intake homepage, you will be asked to choose either Lakeland Regional Health Medical Center or Lakeland Regional Health Physician Group.

Choose Lakeland Regional Health Medical Center if requesting:

  • Hospital Records
  • LRH Team Member COVID-19 Testing Records
  • City of Lakeland Employee COVID-19 Testing Records
  • Drive-Thru COVID-19 Testing Records

Choose Lakeland Regional Health Physician Group if requesting:

  • Doctor’s Office Records
  • Respiratory Care Center Records

When requesting medical records electronically, you must provide an email address and photo ID.

By Mail-

(Radiology CDs can only be received by mail.)

To receive your records by mail you will need to fill out an authorization form. This form can be accessed here. You can send this form to Lakeland Regional Health by using one of the options below:

Please allow 5 to 7 days for delivery via USPS for medical record copies.
Please allow 7 to 10 days for Radiology CD delivery via USPS.
