Take charge of your health with our Diabetes Prevention Program, designed to help you make lasting lifestyle changes and reduce your risk of developing type 2 diabetes.
Our FREE program includes:
– Personalized Coaching: Work with expert coaches who guide you every step of the way.
– Interactive Sessions: Participate in engaging group sessions focused on nutrition, physical activity, and stress management.
– Tools for Success: Access resources and tips to help you make healthy choices and stay motivated.
– Community Support: Connect with others on the same journey for encouragement and accountability.
Qualifications include:
• Diagnosis of prediabetes
• Must be 18 years old or older
• Copy of lab work within the last year fasting blood glucose 110-125 mg/dL or A1C test 5.7-6.4
• BMI ≥ 25
• History of gestational diabetes is OK
• No previous diagnosis of type 1 or type 2 diabetes
• Limited # of slots available for participants at risk diabetes per risk test without labs
The new cohort begins in September. This is a yearlong program. Classes will be on Mondays at 5:30 PM at the Kathleen Campus.
